The paperwork behind a CareBridge engagement.

The operating procedure we follow when bringing a practice onto our services, and the agreement that governs how we handle protected health information. Both are available to read before you commit to anything.

Business Associate Agreement

Draft

Word document (.docx)  ·  14 KB  ·  Sections 1–6

The HIPAA business associate agreement between CareBridge and the practice, setting out how protected health information may be used, what is prohibited, the minimum-necessary standard, and the safeguards we apply.

This is a working draft, not a document to sign. It is published for review only. The text is complete through Section 5 and stops part-way through Section 6, and the remaining terms a business associate agreement ordinarily carries — subcontractors, breach and security-incident reporting, individual access and amendment rights, accounting of disclosures, availability of records to HHS, term and termination, and return or destruction of PHI — have not yet been added. A final version will be issued for execution once it has been completed and reviewed by counsel.

What it covers so far

  • Parties and effective date
  • Purpose and scope
  • Definitions
  • Permitted uses and disclosures of PHI
  • Prohibited uses and disclosures
  • Minimum necessary use of PHI
  • Safeguards (incomplete)
